HOW TO REGISTER AT EITHER PRACTICE

If you are moving permanently into either the Milford on Sea or Lymington areas you should register with a doctor at the appropriate surgery.

Please take your NHS Registration Card into reception and ask if you can be added to a Doctors list.

If you have lost your NHS Registration Card you will be asked to complete a form which enables the Practice to ask your previous Doctor to forward your records. A new NHS Registration Card will sent to you in the post.

On registration you will be invited to attend at the surgery for a Registration Interview usually carried out by one of the Practice Nurses. Details will be taken of your medical history, your blood pressure will be recorded along with information about any ongoing treatments and repeat prescription requirements. You will also be measured and weighed to bring your notes up to date.


ABOUT THE PARTNERSHIP

This is a long established Group Practice of five doctors at Wistaria in partnership with three doctors at Milford on Sea Medical Centre.
Both Practices have recently moved into new purpose-built premises and are aiming to offer an increased range of services in the new, pleasant and spacious surroundings.

To support both patients and doctors there are comprehensive Practice teams at both surgeries consisting of Practice and Community Nurses, Health Visitors, Practice Manager, Receptionists and the Administration Support Staff.
